Your first visit, from call to plan
Many patients come because their dentist or hygienist suggested a gum check. You don't need a referral, but some plans ask for one.

Before you come
Humberto checks your plan if you'd like and tells you what he learned. Forms are on our patient portal, not on this website.

At the visit
A talk, measurements around every tooth, X-rays if they're needed and a written plan with an estimate.

Afterward
With your permission we send your dentist a note. You keep going to your dentist for fillings, crowns and routine care.

Your plan card and a photo ID
So Humberto can check your benefits.
The name of your dentist
We ask them for recent X-rays if you've had them.
Your questions
Write them down. Dr. Montes starts by listening.
Someone to come with you
If it helps you remember the plan.
